Students wishing to enroll in Retake-Improvement-Dropped-Merged-Special Courses are hereby instructed to fill-up and submit this online “Enrolment Form for Retake-Improvement-Dropped-Merged-Special Courses” after paying the enrollment fee.

Students will be able to download the Enrolment Report from ERP after 3 days of submitting this form.
